Pattern: when does something get added here?

A new OQ gets added when:

• The operator asks a question in chat that the Mavis team can't answer in-session.
• A doer Mavis hits a decision that requires operator authority.
• A destructive op needs explicit go-ahead.
• A deadline is approaching (PAT expiry, dup-cleanup confirmation window, etc.).

A new PK gets added when:

• A Mavis explicitly puts work on hold with no immediate next step.
• A plan is documented but no lane is actively working it.

The open-questions list is the operator's checklist. The parked list is "things we know about but aren't chasing."
